Job Title: Social Prescribing Manager
Location: Whaley Bridge, High Peak, SK23 7HX but extensive travel required across the High Peak.
Salary: £28,672 pa. Fixed Term Contract until March 2024 possibility of extension dependent on funding.
Working Hours: 37 hours per Week
Responsible For: Lead, develop and deliver a Social Prescribing service across the High Peak (excluding Glossop)
Social Prescribing aims to increase people’s active involvement in their local communities. It can strengthen resilience at both a personal and community level, and it reduces health inequalities by supporting people to address the wider determinants of health. The service is funded through the Primary Care Network (PCN) who are a group of 8 GP practices in the Buxton, Hartington and North High Peak area.
This role is to lead and implement the Social Prescribing service being delivered in partnership between High Peak CVS and the Primary Care Network (PCN). The key aim of the role is to develop the Social Prescribing service across the High Peak (excluding Glossopdale). This service must be holistic in supporting and improving the health and wellbeing of local residents by ensuring that the intervention best meets their bespoke social needs identified in their individual care plans. The role will work closely with local PCN, Community, acute and social care services, to refer clients for social care and support services in the Voluntary and Community sector (VCS), and supporting patients to take responsibility for improving their health and wellbeing.
