The High Peak Social Prescribing Service commenced March 2020, days before the first lockdown! During this time, our team has grown from two to seven and supported hundreds of people throughout the High Peak.
In the past 2 years we have ensured clients have been supported through lockdown and this year alone we have received 326 referrals, supporting clients from the eight High Peak PCN practices!
Social prescribing is a way for local agencies to refer people to a link worker. Link workers give people time, focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing. They connect people to community groups and statutory services for practical and emotional support
