In its third year, the High Peak Social Prescribing service has seen its opening coincide with a global pandemic, experienced reduced capacity and had Covid restrictions enforced and then
lifted, all of which have presented its own challenges and opportunities. Providing high quality support to High Peak clients is the team’s focus, and they have worked tirelessly to make sure that things run as smoothly as possible.
As a team, we have continually proved our dedication over the past 12 months, and over the coming year, we will see even more changes which will position us among one of the county’s largest Social Prescribing teams.

In this report, we present a review of the service over the last 12 months and the data of the previous quarter (January-March 2023) of the year.

The team

Over the last year, the High Peak team has welcomed Nicki whilst saying Goodbye to Neda.

In the summer of 2023, the team will be further expanding to welcome 4 new roles specialising in:

  • Children & Young People
  • Hospital Discharge
  • Adult
  • On Duty Triage.
