An exciting opportunity has now arisen, and we are looking to recruit for a number of positions within our Social Prescribing Team in the High Peak, working with adults, young people and families.
Social Prescribing works directly with individuals to look at what matters to them and facilitates links to external agencies, appropriate services and support from the voluntary, community and non-profit sector. Referrals come mainly from GP surgeries but also other health and social care professionals and schools
Previous experience of working in a charity and/or health care
environment would be beneficial to your application but not essential.
This is a fantastic opportunity to play a key role for an organisation who do
great work.
We have a full time position available but also looking to appoint to a part time post of 12 hours plus where there is some flexibility over working hours.
